Yamamoto is a new character who appears in Saikoroshi-hen. When the physician comes to see Rika in her bed, she first sees Irie but he rapidly changes into Yamamoto. She subsequently learns that the clinic is not named the "Irie Clinic" but the "Takano Clinic," and it was not created by Irie but by Hifumi.
After Yamamoto brings Rika to the clinic, she asks him about Takano Miyo, Irie Kyōsuke, Okonogi Tetsurō and the Hinamizawa Syndrome but he denies any knowledge of any of them or it. The next day, Rika tells him that she is not from this world, but from another where she died and that she can speak with a girl named Hanyū from her last world through a crystal ball. While he finds it strange, he acts as if he believes in her and claims that if he could talk with Hanyū he would try to take her back in her world. However, Yamamoto immediately tells to Mrs. Furude everything about their conversation with the suggestion that Rika is confabulating.
